Incased in an envolope with a legal statement on the back and what looks like a long phone/fax number "#2006-1501-0504-0322" written on the top in pen. Inside contains 4 2D/2D floppy Disks all numbered in thin envolopes, on the back of the cover is a "Diskette care and handling information"
